Practitioner Agreement

I agree to meet the ZCOC minimum practice requirements:

• Attend the Center a minimum of twice per month.
• Attend Work Practice a minimum of once per month
• Attend individual instruction once per month
• Read the email Newsletter each month.

If I cannot meet the requirements due to extenuating circumstances, I will contact the Center.

I understand I may cancel my subscription through Pay Pal at any time. However, if I want ZCOC to cancel my subscription for me, I will contact the Center. I understand no refunds will be given for more than one month’s fees.
